The anophthalmia and microphthalmia are characterised, respectively, by the absence of an eye and a presence of a smaller eye inside the orbit. The condition of a complete absence of an eye globe is very rare: usually, globe residues or underdeveloped bulb forms with no seeing function can be found inside the eye socket.
Anophthalmia and Microphthalmia represent malformations of the eye development, often associated with complex syndromes which also affect other systems.
As far as these malformations are concerned (grouped under the term MAC: Anophthalmia, Microphthalmia, Coloboma) it is maintained that two cases out of three are of genetic nature and are therefore inborn or, in other words, present since birth.
In these pathological contexts, a child is functionally blind (to one or both eyes); however, in addition to the antomical-functional damage, there is also the disparity of the gaze, an essential element of expression in human relations.
To ensure a better development of the eye socket and the surrounding facial structures, and to improve the proportions of the face, it is recommended to insert an expander or conformer: it allows a more symmetrical development of the child’s facial bones which, in adult age, may facilitate the use of a prosthetic eye.

The insertion of the conformer is recommended for all children affected by MAC syndrome who have been diagnosed with Anophthalmia/Microphthalmia under narcosis without a possibility of sight recovery.
